What to bring with your application
Bring copies of everything below so we can review your file right away. Missing a piece? Reach out — our team is happy to help.

Current Completed Application
The MHA Headstart application form, filled out and signed.

Birth Certificate
A copy of the child's birth certificate.

Medical Insurance
A copy of the child's current insurance card.
- 4
Tribal ID (extra points)
A copy of the child's Tribal ID when applicable.
- 5
Special Services
IEP, IFSP, referrals, OT / ST paperwork — anything relevant to your child's care.

Current Health Physical / Blood work / Immunizations
Up-to-date health records from your provider.

Income Documents
Recent check stub, direct deposit letter from employer, W2, or 1040 tax form.
